Good day sir/maam. Request permission to track my serial number because i am planning to join the arescom..my certificate got lost after a smokers night happens that time somewhere 2010-2011..and now i'm interested whether i can claim my serial number or its already deactivated. Thank you. Ps: i have 4 certification at me if you want some verification or evidence. It just happen that my certification which my serial number attach was lost...email me at [email protected]
October 24, 2022 Dear Joshua Adam, Greetings! Thank you for your request dated Oct 22, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You may proceed to or call your school/ROTC unit/Reserve Center where you finished your ROTC program. Present your authenticated Certificate of Completion to the Admin and Personnel Officer of the unit to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
