Under Section V 2(a) of the LCWD FOI Manual, requestor shall fill out the FOI Request Form and submit the same to [email protected]. Thus, our FOI Receiving Officer (FRO) contacted you on August 9, 2023 thru your provided email address [email protected] informing you of the said requirement and requested also to submit another valid ID. Several follow ups were also sent via SMS and email dated August 10, 14, & 16, 2023. Our FRO also called you on August 16, 2023 however, a personnel from your office informed us that you were on sick leave and will just relay to you the requirements as he is also aware of the FOI process. Please be informed that this Office is already processing your request despite absence of the lacking requirements. This will only be released once the said requirements were submitted to this Office or thru our official FOI email address [email protected].
