Good Day, Ma'am /Sir: I am Shandler Dwight A. Iso and I am writing this message to request from your good office, to inquire for my ROTC/NSTP serial number. I need the serial number for academic purposes. I took my ROTC classes in Negros Oriental State University, Dumaguete City. I graduated my ROTC classes in 2015 but graduated my degree in the year 2019. Hoping for your kind consideration. Respectfully, Shandler Dwight A. Iso
December 22, 2022 Dear Shandler Dwight, Greetings! Thank you for your request dated Dec 22, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Request for National Service Training Program (NSTP) Serial Number. Response to your request We regret to inform you that this is not a valid FOI Request. You may proceed to or call your ROTC unit/Reserve Center where you finished your ROTC program. Present your authenticated Certificate of Completion to the Admin and Personnel Officer of the unit to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you. Good day Sir: I have been in contact with my university however, they have lost the copies of the list of the 2015 graduates. They said that they will be making a letter of request to the higher office however, they have not responded to my inquiries or updates and it has been more than a month.
