To Whom it may concern, Good day, I am Johanna Marie B. Cabanisas, with passport number P4552084C. I would like to please request for my Student Permanent Record from Our School Of Our Lady of La Salette (Bulacan) for my Grade 7, First City Providential College for my Grade 8 to Grade 10, and University of Santo Tomas for my Grade 11 to Grade 12. It will be use for my enrollment in Malaysia as international student. I am humbly requesting for your approval and to release it as soon as possible, preferably before end of month. You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
