Good day! I would like to request for an e-mail address should further details be needed given word limit.  I would like to request for information on physical activity (PA) of children with disabilities (CWDs), such as: % of CWDs who: 1)engage in at 60 min of moderate-to-vigorous intensity PA per day or at least 4 days / week; 2)average percentiles of CWDs for physical fitness indicators; 3)engage in unstructured/unorganized active play OR are outdoors for more than 2 hrs / day; 4)use active transportation; 5) % of family and peers who are facilitate PA and sport opportunities OR are active.

Response to your request

While our aim is to provide information whenever possible, in this instance, we find your request to be incomplete. Thus, your FOI request did not meet the requisites of a valid FOI request. 

Section 9 of Executive Order No. 2 states that:
SECTION 9. Procedure. The following procedure shall govern the filing and processing of request for access to information:
(a) Any person who requests access to information shall submit a written request to the government office concerned. The request shall state the name and contact information of the requesting party, provide valid proof of his identification or authorization, reasonably describe the information requested, and the reason for, or purpose of, the request for information: Provided, that no request shall be denied or refused acceptance unless the reason for the request is contrary to law, existing rules and regulations or it is one of the exceptions contained in the inventory or updated inventory of exception as hereinabove provided. X X X 	

As emphasized, you only enumerated the requested information and purpose of the request, but, failed to state your contact information. Lastly, we noticed that you did not attach any valid proof of identification. Therefore, we constrain to deny your request.

You may wish to submit a new request and take note of the requisites next time you do.
