I'm Rolando R. Simon Jr. , I am a bona fide member of the Social Security System (SSS) with assigned SSS Number 07-2988422-2. I was an employee of Flexi Finance Asia Inc., with office address at CIFC tower 2nd floor, Mabolo Cebu City from April 8, 2019 to September 15, 2020. I was terminated from employment effective September 15, 2020 due to Pandemic/Covid-19. I cannot secure a Notice of Termination from my employer. I would like to request a Certification for SSS to claim for benifits. Thank you 🙏😊

October 16, 2020 Dear Rolando Simon Jr, Greetings! Thank you for your request dated Oct 15, 2020 11:22:58 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request for Certification of involuntarily Separation for SSS. Response to Request This is to inform you of the process and requirements by which to apply for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION for SSS issued by DOLE-NCR. An employee - Local or Overseas Filipino Worker (OFW) who was involuntarily separated by their employer may file their application for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION. The requirements for the issuance of the certificate of involuntary separation for uploading online thru email at [email protected] or [email protected] as follows: a. valid email address (google account) b. at least one (1) valid I.D. (scanned copy) c. scanned copy of the Notice of Termination issued by the employer, in the absence of Notice of Termination, an applicant may submit a duly notarized Affidavit of Termination of Employment For further assistance, you may call DOLE hotline 1349 or call DOLE-RO 7 at (032) - 266-9722 Telefax (032)-416-6167 or you may refer to attached Department Order No.1 series 2019, visit the FAQ section of the eFOI site: https://www.foi.gov.ph/help for details.
