Hi Dole, I would like to follow up on my request for Certificate of Involuntary Separation for SSS (LOCALLY EMPLOYED ONLY). I sent the first request on July 4, 2021 but didn't get the certificate or any update from you so I thought there's something wrong with email I use as I used Gmail email account [email protected] so I decided to send a request again on June 16, 2021 using my Yahoo [email protected] but still didn't get any response. I was involuntary separated from my company on July 1, 2021. Hoping for your quick response on this matter. Sherryl Ardaniel

July 22, 2021 Dear Sherryl Ardaniel, Greetings! Thank you for your request dated Jul 19, 2021 10:47:33 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for Request for Certificate of Involuntary Separation for SSS (LOCALLY EMPLOYED ONLY). Please be advised that pursuant to the Department Circular No 01. Series 2019-Section IV- Venue of Filing of Application for DOLE Certification stated that "employees who are involuntary separated may file their application for DOLE certification at the DOLE Field Office of jurisdiction". Separated employees may apply via walk-in but due to the pandemic, affected employees are highly encouraged to apply online if the service is available.
