Your Request You asked for Request for Certificate of Involuntary Separation for SSS. Response to Request Your FOI request is approved Below is the process of the application for DOLE e-Certification for Involuntary Separation. LOCAL EMPLOYEES who are INVOLUNTARILY SEPARATED/PERMANENTLY TERMINATED due to any of, but not limited to, the following reasons may qualify for the electronic certification of DOLE as part of the processing of Unemployment Benefit Claim through the SSS Website: • Installation of labor-saving devices; • Redundancy; • Retrenchment or downsizing; • Closure or cessation of operation; • Disease/Illness of the employee whose continued employment is prohibited by law or is prejudicial to his or her co-employees' health; • Economic downturn • Natural or human-induced calamities/disasters; and • Other analogous cases as may be determined by DOLE and SSS. Transaction Number provided in the SSS e-mail notification together with the screenshot of the e-mail notification. 2. One (1) valid identification document with signature and photo 3. Copy of Notice of Termination of Employment issued by the employer or duly notarized Affidavit of Termination of Employment, in the absence of Notice of Termination of Employment, and 4. Certificate of Pending Case and/or Police Report, if applicable DOLE shall evaluate your application based on the existing guidelines which includes the verification and confirmation of details indicated in the supporting documents submitted against the Establishment Report submitted by the employer/company to respective DOLE Offices.
