Hi I would like to Request for Certificate of Involuntary Separation for SSS on applying the unemployment benefits claim in SSS.

April 11, 2022 Dear Dioscoro Yangson, Greetings! Thank you for your request dated Mar 22, 2022 03:17:45 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Kindly file your Request for Certification of Involuntary Separation on-line by visiting our website https://clients.ncr.dole.gov.ph/home/ and click the REQUEST FOR CERTIFICATE OF INVOLUNTARY SEPARATION FOR SSS. Applicant is required to attach at least one (1) valid I.D. and a copy of the Notice of Termination issued by the employer. In the absence of Notice of Termination, an applicant may submit a duly notarized Affidavit of Termination of Employment pursuant to Social Security System (SSS) Circular No. 2019-11.
