Good Evening!

I would like to seek for your kind assistance if you could help me through cash assistance on any amount heartily given. 

I was a Covid-19 Positive patient from the period of August 16, 2021 - September 1, 2021. Eventhough I had surpassed the desease, financially I had struggle since I am the breadwinner of my family. 

I had no father already who pass last 2018 due to liver cancer, my mother and sister had Diabetes and are already maintaining medicines and suppliments for it. I also have a highschool sibling that needs my support.

Hi po, nagbabasakali lang po, kahit kunting tulong walang wla na po tlga kami, buntis po ako at manganganak na this month  na po, kaya hirap tlga, wla pa gamit bb ko, delivery rider lng po asawa ko. frontliner  po ako at isa sa nag sacrifice at na walan ng trabaho ng ma buntis. Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have all the information you have requested. However, if you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you.

The forms are available at the SSS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center
3. Medical Certificate or Home Quarantine Clearance issued by LGU Health Office/BHERT
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
