Hi Ma'am/Sir, Good Evening! I would like to seek for your kind assistance if you could help me through cash assistance on any amount heartily given. I was a Covid-19 Positive patient from the period of August 16, 2021 - September 1, 2021. Eventhough I had surpassed the desease, financially I had struggle since I am the breadwinner of my family. I had no father already who pass last 2018 due to liver cancer, my mother and sister had Diabetes and are already maintaining medicines and suppliments for it. I also have a highschool sibling that needs my support. Hoping for your positive respon

November 3, 2021 Dear Junrey Tinoy, Greetings! Thank you for your request dated Oct 28, 2021 08:26:29 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request for cash assistance due to civid-19 tested positive. Response to Request Your FOI request is approved. We would like to inform you that we don't have Cash assistance for COVID-19 patients. If you are an employee who suffered from a work-connected COVID, You can file your EC sickness claim at the SSS branch nearest you(for private sector employees); or at the GSIS (for public sector employees). The forms are available at the SSS/GSIS/ECC website. EC Claims Requirements: 1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19. 2. RT-PCR or Swab Test Result (Positive) 3. Medical Certificate or Home Quarantine Clearance 4. 2 Valid IDs Thank you and God bless Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Dr. Rosana P.
