Ms. Villaruel be correctly informed that requests related to issuance of Vaccination Certificate/Card is a function of the Department of Health through its accredited clinics and hospitals. The Overseas Workers Welfare Administration is mandated by law to provide welfare assistance and benefits primarily to OFWs in distressful situation. Hence, it is strongly suggested that you visit the DOH website on your concern or coordinate with the clinic/hospital that provided the vaccination shots. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing the OWWA- FOI Decision Makers at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter.
