Hi, Good day. I am Christian Jeremey Lopez and I humbly request a copy of my ALS certificate. I would like to continue study in college this upcoming school year and I need the certificate as a requirement for admission. I've taken the ALS examination at Navotas Elementary School Central back in 2008 or 2009. I was given a copy of the certificate before, by the DepED Division office in Malabon City but it has been washed away by a flood due to heavy rain several years ago. I am willing to pay any fees that this request may incur. Thank you. Hoping you could get back to me.
