Hi, I would like to request for the following: My Transcript of Records for schoolyears 2001-2005 from Saint Vincent De Paul College (1st year - 4th year highschool). located at Bagtikan St. Meadwood Exec. Village BACOOR, Cavite 4102 St. Vincent De Paul College Bacoor Cavite has closed in 2022 and I'm hoping that academic transcripts have been forwarded to the Dep't of Education. Thank you for your assistance.

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
