Respected Sir/Ma'am, I am Janet Del Rosario Tait, currently residing at Hilongos, Leyte. On May 14,2021, I lost my phone at a convenience store. The mobile is Vivo Y11 and the color is mineral blue. The phone has 2 sim IMEI numbers and the IMEI 1 is 861238053369132 and IMEI 2 is 861238053369124 and the serial number is Y906TJS00A00. Approximate value of the new device of this model is Php 5,999.

Your Request You asked for Report lost/stolen phone. Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information or email at [email protected] . The following are the requirements: 1. Duly accomplished and notarized AFFIDAVIT OF OWNERSHIP AND LOSS WITH UNDERTAKING [Form No. NTC 1-24] Note: The Affidavit of Loss must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1.
