Response to your request In reply, kindly proceed to the Revenue District Office (RDO) where you are registered, in your case, you are registered at RDO No. 41-Mandaluyong City. For further assistance, you may call the Client Support Section (CSS) of RDO 41 at telephone numbers (02) 8531-0691 and (02) 8532-5303. In case of re-issuance due to damage or lost TIN Card, an Affidavit of Loss and P100.00 replacement fee shall be submitted/paid in the RDO. Thank you.
