Hi Sir/Maam. Scholar po ako ng PVAO for 36 months sa Mapua Institute of Technology sa kursong BS Chemical Engr. at parents ko muna nagbabayad ng matrikula ko. Kaso meron pa po kame hindi nakukuha or collect na cheque. Paano po kaya yung proseso ng pagcollect ng cheque. Heto po yung Claim No. EB-MCO-02-031580. Salamat.

Upon verification with our Claims - Educational Section. The last payment you have received prior to your matriculation dues was in 2004-2005. And no existing records following up to this date.  

In line with your request, the Educational Section would like to know the year you continued your enrollment and if it's under the same school and course. Kindly provide us with the details of your matriculation and other requirements pertaining to your enrollment, so we can process and forego your request for the unclaimed matriculation due you have incurred.
