Good Day! Praised be Jesus and Mary. I'm Ryan Joseph S. Sabardan, a registered pharmacist and currently a student of Clinical Pharmacy in University of the Immaculate Conception. We are currently working on our thesis proposal and we chose to research on the Prevalence of Health Literacy among Adults in Rural Area in Agusan Del Norte. We found the research on the Prevalence of Limited Health Literacy in the Philippines: First National Survey and we would like to request a copy of the survey form (adapted Asia version of the European Health Literacy Survey Questionnaire - 47.
