Please be informed that your requested information on PPG No. 256-14 (Eligibility of Surviving Spouse to Survivorship Benefits Amid Issues of Dependency and Contrary Claim or Information) is now available for pick-up at the FOI Booth, Level 3, GSIS, Pasay City. In this regard, the requested information shall be released to you personally or to your duly authorized representative upon payment of applicable fees and presentation of the following requirements: 1. If for pick-up by requester: any government-issued ID of the requester. 2. If for pick-up by duly authorized representative: a. Authorization letter duly signed by the requester; b. Photocopy of government-issued ID of the requester; and c. Government-issued ID of the authorized representative (original to be presented; photocopy to be submitted). We would like to inform you that the cost of processing for the information requested is Php 55.00 only (covering the reproduction and copying of information per GSIS Revised FOI Manual).
