I would like to request an information regarding Rosario Marcelo Driz a former member of phillipine soldier during the time of world war 2.i am Maria Simon Driz the youngest daughter,nais ko po sana malaman kung may posibilidad po ba na magkaron nang pensyon ang aking ama sa kanyang paglilingkod sa bayan nuong mga panahong nabubuhay pa siya.sinubukan nang aking ama i proseso ang kanyang veterans right of pension nuong nabubuhay pa siya ngunit inabot na lamang siya nang kamatayan na hindi ito naisakatuparan at walang development...nais ko po sana itong ipag patuloy.sana matulungan ninyo ako

Amount
• Php 20,000/month if Senior War Veteran;
• Php 5, 000.00 if Post War Veteran and Surviving spouse
Limitations
1. The widow-beneficiary should not be receiving a similar pension for the same consideration from other consideration from other government funds or from the United States government.
2. The pension is terminated when the surviving spouse remarried
3. Veteran died between 09 April 1990 and 09 April 1994 before his 65th birthday: Effective date is widow’s 65th birthday or 09 April 1994, whichever is earlier, but not earlier than veteran’s date of death.
4. Veteran died between 09 April 1990 and 09 April 1994 after his 65th birthday; Effective date is veteran’s date of death.
5. Veteran died after 09 April 1994: Effective date is veteran’s date of death.
Basic requirements (Pangunahing dokumento)
1.Proof of service
• For WWII AFP veterans – Military Service Record from NRD, OTAG. Expc serviceman – MSR from Camp Crame
• For Philippine Scout – enlistment record/report of separation from USVA. Cut-off date for the Philippine Scout Service (on or before 3 June 1946)
* Claim number of any approved claim, if dg.
2. Proof of birth
• Birth certificate from LCR/NSO/archives office with registry no. (original or certified photocopy done by issuing officer)
3. Proof of death
• Death certificate from LCR/NSO with LCR registry number (original or photocopy)
4. Proof of marriage
• Marriage certificate from LCR/NSO with registry number (original or certified photocopy).
5. Valid ID (passport, driver’s license, COMELEC ID, postal ID, GSIS/SSS ID, AFP ID or VMMC greencard).
6. Two (2) pcs. 2×2 latest ID pictures of veteran and spouse
Secondary requirements in cases basic documents are insufficient:
(karagdagang dokumento kung hindi sapat ang pangunahing dokumento na isinumite ayon sa nakatala sa itaas o kawalan ng pangunahing dokumento)

a. Discharge certificate
b. Form 23/21/24
c. Affidavit of 2 comrades-in-arms (in cases information in MSR is insufficient)
2. Proof of birth
a. Certificate of non-availability issued by LCR/NSO or archives office
b. Baptisimal certificate
c. Joint affidavit of 2 disintererested persons
d. Certificate of 2 disinnterested persons
3. Proof of death
a. Certificate of non-availability issued by LCR/NSO or archives office
b. Joint affidavit of 2 disinterested persons
c. Burial permit or certificate of interment record
d. Casualty report
4. Proof of marriage, submit any 2 of a to e
a. Certificate of non-availability issued by LCR/NSO or Archives Office

I refer to my letter of Feb 16, 2022 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Pilipino veteran soldier of world war 2. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of May 11, 2022.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
