Hello, I would like to request a copy of my PhilHealth Contributions from December 2020 to March 2021 reflecting the remark as "Separated" as erroneously reported by my employer for them settle any outstanding balance and also for legal purposes as artifact for illegal dismissal. (NLRC Case: NCR-03-01093-21) PhilHealth website does not show payor nor the reason why I have unpaid month/s. Please contact me if additional information is needed to obtain this request. If request cannot be accommodated, any certification from your office of the month and reason will suffice.
