Hello, I would just like to inquire about how I can register for a PhilHealth number online for employment purposes. Thank you.
October 16, 2023 Dear Carlos Miguel, Greetings! Thank you for your request dated Oct 13, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for PhilHealth Number. Response to your request Due to the information security incident encountered by PhilHealth last September 22, 2023, we apologize that affected systems needed to be temporarily disabled or unplugged as part of our containment measures. Thus, per PhilHealth Advisory No. 0034, Series of 2023, we advise the public to register with PhilHealth through the Local Health Insurance Office (LHIO) in your area. The requirements are as follows: 1) Two (2) copies of properly accomplished PhilHealth Member Registration Form (PMRF; 2) Valid ID; and 3) Supporting documents (as applicable): a) Original and clear copy of Birth Certificate or Marriage Certificate; b) Originally signed and duly accomplished Report of Employee-Member-ER2 from your company (if currently employed); c) Original and clear copy of the following (if declaring qualified dependent/s): - Marriage Certificate - Legal Spouse who is not a member/an active member - Birth Certificate - Child/Children/Foster child 20 years old and below - unmarried/unemployed - OSCA ID and Birth Certificate of member/registrant- Parents 60 years old and above Kindly bring a valid government-issued ID as proof of identity. Moreover, if transacting through a representative, please provide the following documents: 1) Signed Authorization Letter from the member indicating the full name, address, contact number, and relationship to the representative; 2) Photocopy of two (2) valid IDs of the member; and 3) Original and photocopy of two (2) valid IDs of the authorized representative. The concerned LHIO shall advise registrants to pick-up their Member Data Record (MDR) and/or PhilHealth ID card once they are already available. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
