1-Hi..I would like to change the claimant of my approved Dole Akap program. 2- From Rizaldo Ynclino/099943509021 to Samantha Isabel Ynclino/09214785064. 3- I am waiting for that for a year without any financial assistance from government in time of pandemic kindly consider my request as a OFW.

March 28, 2022 Dear Mr. Rizaldo Ynclino, Greetings! Thank you for your request dated Mar 28, 2022 12:13:09 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of a change of claimant to the DOLE-AKAP Application. Mr. Ynclino the eFOI is asking for a government issued ID not your COVID-19 Vaccination Card. Hence, we cannot check details of your DOLE-AKAP. With request for the change of claimant you must personally coordinate with the DOLE Regional Office where you submitted your application for DOLE-AKAP Call DOLE Hotline 1348 para sa tamang balita tungkol sa DOLE-AKAP. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
