Greetings! I am Sean Michael A. De Vera, Iwas graduated Senior High School on April 2018 at La Paz National High School La Paz - Tarlac province. After finishing high school, I decided to Enrolled in Midway Colleges located at 162, Bitas, Maharlika Highway, Cabanatuan City, province of Nueva Ecija, Philippines. However, I stop and dropped all of my subjects in 2018 and My Original Form 137 has been sent to Midway Colleges. With that, I would like to request if I can obtain my original form 137 because I need it for Application for Graduation in my current University. Thank you!
