Hi, I am John Kelly B. Bergonio. I am requesting for my NSTP Serial Number from my former school ACLC NAGA CAMPUS from Naga City, Camarines Sur. The serial number will be use to release my TOR and the TOR will be use for LET exam application this coming March 2023. I took the NSTP way back 2008-2009 school year both NSTP 1 and NSTP 2. They will input the serial number in my TOR. Thank you and I hope that my message will be notice.

Thank you for your request dated Jan 17, 2023 06:32:44 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for your NSTP SERIAL NUMBER. Response to your request Please be advised that per Implementing Rules and Regulations of the Republic Act No. 9163 or “National Service Training Program (NSTP) Act of 2001”, the Higher Education Institution (or your school) is responsible for securing the NSTP serial number upon submission of the NSTP Enrollment List and request letter, duly signed by the NSTP Director and/or registrar. Given these, may we advise you to contact your school registrar where you finished your NSTP to help you on this matter. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
