Hi! I would like to request a Notice of Involuntary Separation from your good office as my Employer was not able to provide me the said document. I have with me only an Affidavit of Separation from Employment and Valid Id and still lacking the Notice of Involuntary Separation to be filed to SSS. Hope for your prompt action. Regards, Mark
