Good day! I would like to request for the Hospital Safety Indexes (DOH-initiated and self-assessment) of Region 1 hospitals. To preserve confidentiality, we request that the hospitals be designated as A, B, C, etc. Also, please remove any other information that may cause the identification of these hospitals (e.g. address, phone #, etc). If available, I would like to request for the indexes for the years 2013 up to 2018. Kindly share and indicate the assessment tool used, too. These will be used as primary sources when analyzing the vulnerabilities/capacities of Region 1 Hospitals. Thankyou!
