You requested for the following information: “NHTS Questionnaire Tool
.”

Unfortunately, the DSWD cannot give the same via the FOI.

DSWD Administrative Order No. 2, series of 2019 entitled Guidelines on the Sharing of Data Generated from the National Household Targeting System for Poverty Reduction [DSWD A) 2009-002] provides: 

4.6. The questionnaire, or the Household Assessment Form (HAF), used to collect household information can be used by other government and non-government agencies only under authorization of and strict supervision from DSWD. The rules, methodology to collect information, and processing that are also defined by DSWD will be used accordingly to maintain the quality of information collected.

Thus, you may visit DSWD NHTO for information whether an individual can access the NHTS Questionnaire. Further, this requires signing a Memorandum of Agreement with the DSWD.
