Dear Sir/Madam, Good day! I'm Ma. Elaiza Jane M. Leorna a former BS Tourism Management student of University of Caloocan City. Based on the records of my previous University, I satisfactorily complied with all the requirements of NSTP Program during the school year 2014-2015. In connection thereto, may I request for the assignment of my NSTP Serial Number.

Thank you for your request dated Jan 31, 2020 01:27:05 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for National service training program department. Your FOI request is approved. Please be advised that per Implementing Rules and Regulations of the Republic Act No. 9163 or “National Service Training Program (NSTP) Act of 2001”, the Higher Education Institution (or your school) is responsible for securing the NSTP serial number upon submission of the NSTP Enrollment List and request letter, duly signed by the NSTP Director and/or registrar.
