I, Zandra O. Mangune, am currently taking up Master of Arts in Education (Major in Mathematics Education) in the University of the Philippines, Diliman. At present, I am gathering data for my study in order to conduct a comprehensive analysis of the mathematics education in the country. Part of the study is to develop learning materials and design an instructional approach that will contribute in enhancing the performance of our high school students in mathematics.
In this connection, I am requesting information regarding the Results of National Achievement Tests of the Fourth Year high school/ grade 10 students in mathematics, covering the period from 2012 to 2018. I am including a short abstract of my study for your reference.

ABSTRACT OF THE STUDY
Thesis Title: Self-Learning Algebra Worksheets: Effects on Students’ Procedural Fluency and Self-determination
This study aims to investigate the effects of the Self-Learning Algebra Worksheets Approach (SLAW) on students’ procedural fluency and self-determination. Specifically, the study seeks to answer the following questions:
1. Do students exposed to SLAW Approach have better skills than those exposed to Conventional Approach in terms of
1a. procedural fluency
1b. self-determination
2. Is there a relationship between students’ procedural fluency and self-determination?

I refer to my letter of Jun 21, 2019 05:31:04 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for National Achievement Test Results.
Please be informed of the additional requirements you need to submit. To process your request, kindly submit the following documents to [email protected], stating your FOI request tracking code:
a. Letter specifying the purpose of the request and the data being requested (year administration, scope, variables);
a. School ID to show proof that you are enrolled in a higher education institution;
B. Copy of your approved thesis/dissertation proposal signed by your Adviser;
c. Endorsement letter signed by the school Dean.
Note that the Bureau of Education Assessment (BEA) can only release 10% of the actual number of examinees per year. Individual test results with names shall not be released unless the “data subject agrees to the collection and processing of personal information about and/or relating to him/her” per Section 3 of Republic Act 10173 or the Data Privacy Act of 2012.
Additionally, a corresponding amount shall be charged by BEA for the processing of the data request.
For more information, kindly refer to Section 14 of DepEd Order no. 55, s. 2016, or the “Policy Guidelines on the National Assessment of Student Learning for the K-12 Basic Education Program” through this link: https://www.deped.gov.ph/wp-content/uploads/2016/06/DO_s2016_55-3.pdf.
