The said documents will be used for my research study in Master's Degree as bases on what particular method I could apply to generate a support learning tool for Self Learning Modules in General Mathematics of the students specially not only in Region XII but particularly in the Divison of Cotabato City. I am writing a formal request letter for me to ask a copy of the National Achievement Test of Senior High School for school years 2016-2017 up to 2019-2020. I just want to have the results particularly for Mathematical Ability parts containing the General Mathematics and Statistics and Probability concepts. Currently, I am enrolled at Graduate School of Notre Dame University, Cotabato City this school year 2020-2021.
I humbly need these data for me to establish the reasons why there is a need to pursue with this research study proposal of mine entitled with “Effectiveness of E-SIGNS: A MELC-based Support Learning Tool for SHS Mathematics Core Subjects”. In which my proposed support learning tool E-SIGNS stands for (Explicit - Simplified Illustrating Guides with Noted Steps) which happens to be my intended output. Hence, in order for me to have very strong bases for this innovative undertaking as something that could generally help the students to learn better the SHS Mathematics Core Subjects which is the general objective of my proposal and for the students to adopt with this so called “New Normal Education” due to COVID-19 crisis.
Hopefully you could grant this request for that sole purpose only and nothing more.

Please be informed of the additional requirements you need to submit. To process your request, kindly submit the following documents to [email protected], stating your FOI request tracking code:
a. Letter specifying the purpose of the request and the data being requested (year administration, scope, variables);
a. School ID to show proof that you are enrolled in a higher education institution;
B. Copy of your approved thesis/dissertation proposal signed by your Adviser;
c. Endorsement letter signed by the school Dean.
Note that the Bureau of Education Assessment (BEA) can only release 10% of the actual number of examinees per year. Individual test results with names shall not be released unless the “data subject agrees to the collection and processing of personal information about and/or relating to him/her” per Section 3 of Republic Act 10173 or the Data Privacy Act of 2012.
Additionally, a corresponding amount shall be charged by BEA for the processing of the data request.
