1. Human error details
2. Division/unit in which human errors occurred (e.g. maintenance)

For clarification, we would like to identify the areas of MRT-3 accidents/malfunctions that could be directly affected/caused by human factors and/or errors from the year 2012 to 2022.
The purpose of this request is to fulfill our academic research requirement for our course subject Railway Engineering Research for this semester, and we will solely use the data requested for our background of the study.
