Hi, I would like to request for relevant memos or department orders that would cover the following: 1. Valid use of Form 137 (can it be used by learner for correcting name in birth certificate?); 2. Standard request forms to be used by the school; 3. Standard procedures from accommodating request from a learner/parent to releasing the requested document; and 4. Other relevant memos that may be of help in implementing standard procedures in releasing learner records on a school level. I am hoping for your response. Thank you.
