To Whom It May Concern, Hi, and Good day Ma'am and Sir, This is Ruby B. Rabe, 23 years old and I request Medical Assistance for my medicines because I am still/continue to take and have to follow up check -up every week. I am also a PWD due to Moderate hearing loss and have been diagnosed with Chronic Suppurative Media, as inactive (L). It was found that my ear drum is 70% perforated and almost gone. there are pussies inside the ear, and the form of my hearing bone is also wrong.
