Sorry for the late reply. I was waiting for this letter in my email then I just remembered that eFOI Ph has its own email. And I left my PC at the office. Attached are the CAD6 and CAD7 forms. Also I attached a journal that I had published based on the dengue cases in Cabanatuan City. I already finished my doctorate degree last Feb 21, 2023 and waiting for the graduation rite from Angeles University Foundation. The purpose of the data is for used in updating my dissertation output and possibly write a journal for a newer output and conclusion.
