Good day. I would like to request current information regarding the ff.
1. A list of all the Public Elementary Schools in Antipolo City
2. The contact number and email of the School Heads of each school
3. Name, email and contact number of the School District Supervisor of Antipolo City
4. Population of Grade 1 to Grade 7 students in each of the public elementary schools in Antipolo
I would use the information to ask for permission to conduct my study in the ff. schools. These would help me accomplish my masters thesis. Thank you

To whom it may concern,
I am Luis Albert V. Crisostomo, a student of Master of Science in Medical Laboratory Science enrolled in Our Lady of Fatima University - Quezon City, and conducting a research titled "THE POST-VACCINATION STATUS OF GRADE ONE IMMUNIZED SUBJECTS WITH MEASLES-RUBELLA VACCINE IN PUBLIC ELEMENTARY SCHOOLS IN ANTIPOLO CITY AND JALA-JALA, RIZAL: BASIS FOR PROPOSED RECOMMENDATIONS FOR THE IMPROVEMENT OF VACCINATION SCHEMES". Using an online survey questionnaire, this study aims to determine the status of the children after being vaccinated by the Measles-Rubella Vaccine, and would also determine the perception of the parents about vaccination. This study would help me formulate recommendations to improve the vaccination scheme in the locale.
In order to proceed, however, I would have to ask permission to the School District Supervisor of Antipolo City to conduct the study. Once approved, I would also ask permission to each of the school heads/principal. This is the reason why I'm humbly asking your office to get a copy of the masterlist of the schools together with the contact no. and email of each school head/principal.
I am also asking for the number of enrolled students in each of the schools so that I may determine the appropriate sample size for my study.
