Please help me track or trace the location of my lost/stolen phone or identify the sim card number registered to the phone. Based on my research, you are capable of doing it through IMEI number. When the theft insert new sim card it will automatically registered to the nearest cell tower so you can identify what sim card number it is. I will report all the information coming from your agency to the police so they can proceed to retrieving operation. I still have the box and receipt so I can show you the IMEI number and other details of the phone. This is a very important matter. Please help me

February 26, 2020 Dear Daniel Quinlog, Greetings! Thank you for your request dated February 25, 2020 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request: You asked for Lost/Stolen Phone. Response to Request: Please be informed that the NTC has no capability to locate lost phone. In reference to the above request , may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City, . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
