Purpose: Lost vaccine card

I would like to raise a concern regarding my vaccination record. I lost my vaccination card this april . I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Sindangan, Zamboanga del norte .My first dose was on December 2, 2021 and the second dose was at January 26, 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it.
