Hi!, I would like to raise a concern regarding my vaccination record. I lost my Vaccination Card today in the public place and I wasn't able to look for it. I was vaccinated in Hilongos Leyte, my first dose was on January 5, 2022 and my 2nd dose was on February 9, 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

Is this request offensive?
