I lost my phone OPPO A5S in just a couple of minutes, none of those people who where there want to say something about a phone being dropped. I would like to track my phone again using my IMEI1: 862876042578112 IMEI2: 862876042578104, I still have the box of the phone. I have some important files and emails there, i can't access my emails because it was connected to a number that was on the lost phone. 639152024862 for globe and 639127495477 for smart my available email if needed is [email protected]

April 1, 2021 Dear Levi Barroga, Greetings! Thank you for your request dated Mar 31, 2021 12:45:48 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Lost Phone. Response to Request Please be informed that the NTC has no capability to track/locate lost phones. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form (you may download at www.ntc.gov.ph 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
