Hi i would like to request to replace my lost philhealth id? i dont have any valid id, except my birth certificate and brgy id. can i go to the nearest branch and request for a new id? thanks.

Response to Request To get a new PhilHealth ID, the requester may submit the accomplished PMRF with the attached (1) valid ID at the nearest PhilHealth Office. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to . Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter.
