Hi! I would like to request a police report for my lost passport. I will be needing it for applying for a new one in DFA.

Response to your request While our aim is to provide information whenever possible, in this instance we advice to proceed directly to nearest/concern police station to file a police report of the said incident. The aforesaid police report will be your requirement together with the affidavit of loss to be provided by any law office in your area.
