Purpose: to get a copy of my lost vacination card

I would like to raise a concern regarding my vaccination record. I lost my vaccination card last Novembef. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. I was vaccinated in via Brgy.Quirino Operations Center's COVID-19 Vaccine at Solano, Nueva Vizcaya. My first dose was on November 8, 2021 and the second dose was at November 29, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it,
