Hi I would like to request to locate my stolen phone Last May 28, 2021. It happened at our boarding house located at 721 Carola St. Sampaloc Manila at around 2:30 to 4:00 am. The IMEI of the device is: 863851040639769 Brand of the phone: Reno 2F It contains my important files and digital wallet accounts as well as my contacts. I still have the box of the device in case it is needed. Thank you.

Your Request You asked for Location of my Stolen Phone. Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we suggest you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information or email at [email protected] . The following are the requirements: 1. Duly accomplished and notarized AFFIDAVIT OF OWNERSHIP AND LOSS WITH UNDERTAKING [Form No. NTC 1-24] Note: The Affidavit of Loss must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 2.
