Your Request You asked for Location of Loss/Stolen Cellphones and to search for loss/stolen cellphone using IMEI. Response to Request In reference to the above request, may we refer you to visit/contact our Consumer Welfare and Protection Division (CWPD) for the Blocking of Lost/Stolen Mobile Phone/Unit. Our office is available from Monday-Friday 7:00AM - 6:00 PM. Or you may send your formal written complaint to the CWPD thru: [email protected] or call at 9213251 & 9267722. Please be informed that this office can't locate/search any loss cellphones, however, we could just request it for blocking the IMEI/Serial NUmber of all lost cellphones provided you have the following requirements : For Walk-In clients : 1. Mobile Phone/Unit box indicating the (IMEI) or Serial Number or Official Receipt of your lost cellphone. 2. Two (2) valid IDs (at least one government-issued identification) For Mail, Fax and E-mail: 1. Duly accomplished and notarized form (Affidavit of Ownership and Loss with Undertaking), form is available at NTC Website (ntc.gov.ph) 2. Scanned copy of the Mobile Phone/Unit box indicating the IMEISErial numbers or copy of Official Receipt with complete required details 3. Copy of two (2) valid IDs (at least one government valid identification) Your right to request a review: If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to: GAMALIEL A.
