Response to your request

While our aim is to provide information whenever possible, in this instance this Office will not  provide the requested data due to the Data Privacy Law. However, you may wish to contact  the PDEA Compliance Service at (02) 927-9702 loc. 197 and/or Telefax no. (02) 920-8110 for verification if the practitioner is a license holder.
