24 September 2018 Dear Mr. Noveda: You requested for information on the “(1) List of PWD Medical Rehabilitation Centers in Region 4A and in the Philippines and (2) List of Accredited PWD Centers in Region 4A and in the Philippines.” Unfortunately, the DSWD does not have the information regarding List of PWD Medical Rehabilitation Centers in Region 4A and in the Philippines. Thus, we can only partially grant your request. PWD Medical Rehabilitation Centers are under the supervision of the Department of Health (DOH). The DOH may have the data you need. Hence, you may wish to direct your request to the DOH. You make make another request via this portal. You may also contact the DOH by telephone at 651-7800 local 5001-5006 or via email at [email protected]. Additionally, pursuant to Presidential Decree 1509, as amended, the National Council on Disability Affairs (NCDA) is the national government agency mandated to formulate policies and coordinate the activities of all agencies, whether public or private, concerning disability issues and concerns.
