Hi! I would like to request for the following data:
1. List of LGUs (city / municipality) that (i) requested or are currently requesting to access, and (ii) have already or have not yet been given access to the Listahanan (NHTS Data). Please include requests for access to all rounds of the Listahanan data (from round 1 in 2009/2010, round 2 in 2015, and the current/upcoming round 3).
2. In the above request, please also indicate the date of request (MM/DD/YYYY) from the LGU and the date of release (MM/DD/YYYY) by the DSWD of the Listahanan data.

I refer to my letter of Apr 01, 2022 12:06:47 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for List of LGUs that Accessed Listahanan (and Dates).
While our aim is to provide information whenever possible, in this instance, we find your request to be incomplete. Thus, your FOI request did not meet the requisites of a valid FOI request.
Section 9 of Executive Order No. 2 states that:
SECTION 9. Procedure. The following procedure shall govern the filing and processing of requests for access to information:
(a) Any person who requests access to information shall submit a written request to the government office concerned. The request shall state the name and contact information of the requesting party, provide valid proof of his identification or authorization, reasonably describe the information requested, and the reason for, or purpose of, the request for information: Provided, that no request shall be denied or refused acceptance unless the reason for the request is contrary to law, existing rules and regulations or it is one of the exceptions contained in the inventory or updated inventory of exception as hereinabove provided. X X X
As emphasized, you only enumerated the requested information and purpose of the request, but, failed to state your contact information. Lastly, we noticed that you did not attach any valid proof of identification.
Given the above, may we request the said requirements to immediately facilitate your FOI request thereto.
