Greetings! I am Eileen Gay Coronel, a graduate of BS Development Communication major in Community Broadcasting with student number 2000-48918, would like to request for a list of UPLB students who graduated on 23 April 2005. This list is being asked by the Philippine Regulatory Commission (PRC) Lucena, as one of the requirements for the filing of Application for the Licensure Exam for Teachers. Hence this request. This refers to your request dated 14 June 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for information on the list of UPLB students who graduated on 23 April 2005. Since you did not provide Form F which needs your signature and should be administered under oath as required under the UP FOI Manual, we regret that we are constrained to DENY your request.
