Your Request You asked for License. Response to Request In relation to your request, please be informed that in order for our Licensing Division to verify the examination result, kindly submit through email a formal request letter duly signed by the examinee, indicating the following examination details: NAME: DATE OF EXAM: PLACE OF EXAM: TYPE OF EXAMINATION TAKEN: NAME OF INSURANCE COMPANY (during the time of examination): Also, kindly submit the identification card (ID card) of the examinee and his/her authorized representative for verification. Please note that our Licensing Division only issue certification to the examinee or to his/her authorized representative administered by this Commission. Moreover, request for a Certification of Examination Result may be done through the following options: A. On-Site at the IC Head Office (Manila) B. Online (Payment shall be made online and the Certification along with the Official Receipt shall be sent to your address via courier. However, please expect that the delivery may take a longer time considering that the document will be sent via courier. Kindly refer to the attached documents relative to the process for the issuance of Certification of Examination Result/s.
